need database website

I need a website with a simple layout

I am guessing that i need some sort of database

i will have about 500 to 1000 people that will register with password and fill out a form then be able to edit their details

users will need to be able to search database to search for users that may have documents listed in form above

inquire for more details

pay is 150 but i am flexible

Compensation: will pay using paypal upon completion
